Your privacy is extremely important to Mevident, Inc. ("Mevident"). This Privacy Policy ("Privacy Policy") tells you what information Mevident collects about you and your use of (the "Site"). This policy also explains how Mevident protects that information and our privacy practices.

By using the Site in any manner whatsoever, you indicate your conclusive acceptance of this Policy. This is an important Policy which you must read carefully so that you will understand clearly both our commitment to you and your privacy, how we may collect and how your information may be used.


Profile Information

Profile information is information that can be used to identify or contact you. By entering your personal information into the Site, you consent to Mevident's use of your profile information in accordance with this Privacy Policy.

When Practitioners publish their personal, professional and business information on the Practitioner Directory, they consent for their profile information to be published on the Practitioner Directory that is accessible by anyone visiting the Site. Otherwise, profile information will not be shared with any User, third-party companies or advertisers, unless Users explicitly agree to share their profile information and only for the purpose they agree to. (e.g. providing their profile information to Practitioners of their choice.)

Mevident may use profile information to contact Users by email. Email communications may include information related to specific conditions or topics that interest User, as well as general health and wellness information, wellness tools, surveys and news.


The website may collect anonymous, non-personal information about your use of the website through the use of "cookies." Cookies are small computer files that are transferred with your permission to your computer's hard drive.

Your browser software can be set to reject all cookies if desired. However, users must at least allow for session-based cookies to use the basic website features. Most browsers offer instructions on how to reset the browser to reject cookies in the "Help" section of the toolbar if interested.

Assessment Survey Data

Assessment Survey on the Site asks Users to provide personal, private, family, health and wellness related information. The information provided to the Assessment Survey and the results generated by the Site will be only accessed by the User in their own personal User account, and will not be shared with any other User, unless the User agrees to share and then only for the purposes they agree (e.g. providing the Assessment Survey Information to Practitioners of their choice.)

Be assured that Mevident is committed to protecting the confidential nature of personal information including health information, provided on the Site to the fullest extent of the law. Mevident will create a Wellness Record. We need this record to provide quality services and to comply with certain legal requirements. Under no circumstances will we purposefully disclose personal information to unauthorized third parties unless we reasonably believe the law requires us to do so

Thus, Mevident will not share the Assessment Survey Information with any party in the personally identifiable manner. However, Mevident will publish Aggregated results of the Assessment Survey on the Site.

User Content Exchanged Between a Member and a Practitioner

This Site may facilitate interactions between a Member and a Practitioner through a Wellness Record. However, this Site cannot control the way any Practitioner uses the Personal Information they obtain or create through such interactions.

Practitioners are required to protect the confidentiality and privacy of their Client Users at the level required by state and federal laws, and ethical codes of conduct under our Terms of Use. However, they may break confidentiality under certain circumstances required by the laws, regulations and codes of conducts of the professions. In addition, while Practitioners are required to be compliant with these laws, Mevident cannot guarantee or be responsible for Practitioner compliance. Therefore, if a Member plans to exchange any personal information, including health information, with any Practitioner on the Site or request the Practitioner to provide such personal or health information to the Wellness Record, you must authorize the practitioner to provide such information in writing and should request a copy of the Practitioner's privacy practices. This Site does not facilitate the process of exchanging such documents between a Member and a Practitioner. It is Members' own responsibility to sign the authorization with Practitioners.


Other than the information published on the Practitioner Directory, Mevident will not release Personal Information to any other party in the personally identifiable manner, except: (1) to comply with valid legal requirements such as a law, regulation, search warrant, subpoena or court order such as a physical threat to you or others; or (2) in cases where Users explicitly agree to share Personal Information with Practitioners of their choice. In the event that we are legally compelled to disclose your Personal Information to a third party, we will notify you unless doing so would violate the law or a court order.

If we are a business associate of a Practitioner, then in addition to keeping health information private and secure, we are required to report any unauthorized use or disclosure or security incident to the Practitioner.

Mevident may publish or provide to third parties non-Personal Information about User statistics that does not allow you to be identified or contacted and that is combined with the non-Personal Information of other users ("Aggregate Information").


Mevident takes technological and physical security measures to protect each User's Personal Information; however, Users must protect their own passwords and login information themselves.

Mevident cannot protect a User's Personal Information from a third-party who has obtained their login and password credentials legally or illegally. Users are cautioned to take great care to protect their own passwords.


Mevident uses a variety of security technologies and procedures to help protect your personal information from unauthorized access, use, and disclosure. Our employees have access to this information on a need-to-know basis only and are bound by the terms of an internal Privacy Policy. They are also trained extensively on safeguarding the privacy and security of health information. Any suspected violation is investigated and the employee subject to disciplinary action, including termination if indicated. Any security incident will be reported as required by law.

Section 5. Linking To Other Websites

Mevident may provide links to other web sites that may be a good resource to you. In doing so, we cannot ensure your privacy once you leave the Mevident website. Please be sure to read each website's privacy statement and consider the source of that website's material before proceeding.

Section 6. No Liability

Although we will use all reasonable efforts to safeguard the privacy and security of the information which you provide to us, transmissions made by means of the internet cannot be made absolutely secure. In no event shall we be liable for any direct, indirect, special, incidental, consequential, punitive, or exemplary damages, including lost profits, (even if we are advised of the possibility of such) arising in any way from the information receive by this web portal or any information transmitted via this web portal.

This Policy is not intended to create a contractual or legal right in or on behalf of any one. From time to time, this Policy will be updated or changed. Please review it routinely.
